What to Send With a Referral
Including these up front helps us evaluate and schedule the patient without delay.
- Referral or order with chief complaint or working diagnosis
- Recent diagnostic reports — MRI, CT, X-ray, or other imaging
- Most recent physician’s notes relevant to the condition
- Patient demographics — name, DOB, and contact phone numbers
- Insurance information for the patient
